Local Number Porting (LNP) & Toll Free Number Port-out (RespOrg) Form

Porting refers to the transfer of your phone number(s) from one carrier to another. While the process is straightforward, it requires certain security checks to verify that you are the rightful owner of the phone number(s).

The form below serves as a Letter of Authorization (LOA), allowing us to facilitate the port and contact your new service provider and schedule the transfer of your listed phone numbers.

To proceed with canceling your services, please select a date and time for a review with your account manager. This meeting will ensure a seamless service disconnect and allow us to discuss any existing term contracts or potential fees and penalties associated with your cancellation. We will make every effort to contact you at the selected time or at a reasonable accommodation.

By agreeing, I confirm that I am either the above-named service customer or represent the business as authorized to change the primary carrier(s) for the listed telephone number(s). I affirm that I am at least 18 years of age and that the name and address I provided match the records held by my local telephone company for each listed number.

I authorize TELAIR.NET Ltd. or its designated agent to act on my behalf in notifying my current carrier(s) of my request to change my preferred carrier(s) for the specified number(s) and services. I also grant permission for TELAIR to obtain any necessary information to facilitate this carrier change.

I understand that agreeing to this request does not guarantee that the numbers will be ported if I am not authorized or if the information does not match the account. I agree to pay any outstanding invoices, which may include additional usage or fees related to services used prior to the porting process.
